
Navigating care for a loved one with dementia is not an easy job! People with dementia often have multiple doctors, including specialists, and all too often there is a communication gap between those providers. This fragmented care leads to poor health outcomes and frequent facility stays
Additionally, family caregivers of dementia patients bear significant mental, physical, emotional, and financial burdens while providing care at home with little or no support structure in place.
To address these issues, the U.S. Centers for Medicare & Medicaid Services has developed the GUIDE Model.
Guiding an Improved Dementia Experience (GUIDE) is a Medicare funded program that provides dementia care navigation services for those who are eligible. This program is designed to assist dementia patients and their caregivers in better navigating healthcare and social support.
