GUIDE Model for Dementia Care

Navigating care for a loved one with dementia is not an easy job! People with dementia often have multiple doctors, including specialists, and all too often there is a communication gap between those providers.  This fragmented care leads to poor health outcomes and frequent facility stays

Additionally, family caregivers of dementia patients bear significant mental, physical, emotional, and financial burdens while providing care at home with little or no support structure in place.

To address these issues, the U.S. Centers for Medicare & Medicaid Services has developed the GUIDE Model.

What is the GUIDE Model?

Guiding an Improved Dementia Experience (GUIDE) is a Medicare funded program that provides dementia care navigation services for those who are eligible. This program is designed to assist dementia patients and their caregivers in better navigating healthcare and social support.

GUIDE aims to:

1. Improve the quality of life for people living with dementia
2. Reduce strain on unpaid caregivers through training, education, and support services
3. Enable people living with dementia to remain in their homes and communities
4. Reduce hospitalizations and emergency room use by managing symptoms at home
5. Enhance care coordination between healthcare providers and community support services

What service(s) are you looking for? *

• Personal Care – Your loved one is fairly independent and wants to remain in their home. Could use help with activities of daily living including light housekeeping, meal preparation, bathing, dressing, and grooming.
• Home Health – Skilled medical care by licensed professionals in the person’s home or facility to help them recover from an injury, surgery, or to manage chronic conditions while maintaining independence and avoiding hospital stays. This includes: skilled nursing services, physical therapy, occupational therapy, speech therapy, wound care, and nutritional counseling.
• GUIDE Dementia Support – Guiding an Improved Dementia Experience is a no-cost Medicare program that covers the costs of dementia care navigation services for those who are eligible. This program is designed to assist dementia patients and their caregivers in better navigating healthcare and social support.
• Hospice – Care for people with life-limiting illness with focus on quality of life rather than curative care. Hospice includes emotional, social, respite, and spiritual support for both the patient and their family.
• Supportive (Palliative) Care – In-home nurse practitioners and nurses focused on promoting quality of life and relieving the symptoms (pain, loss of appetite, nausea, anxiety, etc.) that arise, especially during treatment for a chronic or serious illness
• House Calls – In-home physician, nurse practitioner, or physician assistant visits for acute needs and/or chronic care management.
